In the United States, water fluoridation remains a widespread public health measure, with about 73% of the population receiving fluoridated water. Fluoridation began in the mid-20th century after studies showed communities with naturally occurring fluoride in their water had lower rates of dental cavities. By adding small, controlled amounts of fluoride to public water supplies, health officials achieved significant reductions in tooth decay, as fluoride strengthens tooth enamel and promotes remineralization. Biochemically, fluoride also influences bone tissue, with small amounts stimulating bone formation, though excessive exposure can cause dental and skeletal fluorosis.
The FDA regulates fluoride in bottled water with limits of 0.7–1.2 mg/L, while the EPA enforces a maximum contaminant level of 4 mg/L for public water systems to prevent adverse health effects. While generally considered a safe and cost-effective strategy for improving oral health, fluoridation has sparked debate about individual choice and potential overexposure. It remains an important part of efforts to reduce disparities in dental health, especially for populations with limited access to dental care.
